Before you worry about anything else, you need the Weapon Modding skill. This is a skill that any character can unlock as they earn skill points each time they level. Every weapon mod in the game has a modding level associated with it. So, for example, a Modding Level 3 Scope requires that your character has level 3 modding and that the weapon has a scope slot.

Once you have a character with at least 1 Weapon or Armor Modding level, simply select a weapon or armor from your inventory and then choose the modify option.
